The definition of hardcore certainly varies.
I personally do not consider any ego shooters, and most other 3d games hardcore whatsoever, as my definition of hardcore isn't based on violence, but more on difficulty.
For example I particularly enjoy simpler things that are so gruesomely difficult and inaccessible that nobody but a handful of people can even barely beat them. Prime examples of that are things such as SMB hacks (usually the japanese hacks, seeing how western players do generally have a rather negative and lacking interest in abusing glitches), that generally involve a lot of skill that not even the original game was intending to be challenged at any time.
