Transformation into 4 characters

Hi guys, at first: sorry for my (sometimes) bad english, i'm german...

My problem:

i created 5 characters and the first one transforms into one of the four other ones.
1.) Is that possible ?
2.) How ?
I dont want to do a temporary transformation. I tried to copy the transform-part of LouisEX but it doesn't work.

Could you please give me an example of one transform-move, e.g. a transformation into the char with ID 12. That would be very nice
You don't have to write sth complicated, but it should work

edit: I'm not very good at data-changing, so reading a transformation-tutorial didn't make me cleverer.^^
