well, the problem is that hp-recovery is set in the exe. You'll have to change the hex to achieve an hp-lost but then you'll lose the healing ability.
Another solution would involve an itr that'll hurt the drinking character. I'm not sure if that's possible but it should be, though :P

Maybe you could try state 18, then you have the burning smoke though.
I can think of a possible way, but this would require to edit every char...

The char has in the drinking frames another body with some special coordinates like y: -1000 and the bottle creates in the drinking frames (dunno the frame number) the object at the same y value like the special body.
The object hurts the drinking character with state 18.

I still think that even if you edit all the characters, you still don't need to opoint an object, I guess it would be enough just to set the itr to approximately that area while using the burning-smoke-hidden-thingy
